29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    A:  On or about 05/24/11 during steel erection activities, the employer did not ensure that fall protection devices or systems were used when accessing and egressing from a second floor area that was 15.5 feet above the ground.        B:  On or about 05/24/11 during steel erection activities, the employer did not ensure that employees used fall protection systems or devices when working on an unprotected deck side that was 15.3 feet above the lower level.

B:  On or about 05/24/11 during steel erection activities, the employer did not ensure that continuous decking was in place so employees were not exposed to deck openings when accessing and egressing from a third floor area that was not continuously decked.
