29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices:\r\n\r\n(a) On or about June 16, 2011, and at times prior, Concrete Express, Inc., as exposing and controlling employer, did not ensure its own employees and the employees of Rocky Mountain Signing, were protected from roadway hazards at the intersection of South Wadsworth Blvd. and Highway 285 in that:\r\n\r\n1) The \"Road Work Ahead\" sigh was not visible one mile south of the work zone to notify north bound Wadsworth Blvd. motorists per the MHT.\r\n\r\n2) No signs alerting motorists of the work zone were posted on side streets that entered the work area per the MHT.\r\n\r\n3) No sign was posted in the north bound taper on Wadsworth Blvd. alerting motorists to the flagger working ahead per the MHT.\r\n\r\n4) Workers on foot entered live lanes of traffic. \r\n

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:\r\n\r\n(a) On or about June 16, 2011, and at times prior, Concrete Express, Inc., did not ensure its employees were adequately trained to recognize hazards of live traffic in the roadway where the employees were assigned to work.
