29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace: (a) Concrete Express Inc, at US Highway 36, Louisville, CO: On and preceding 12/3/15, the employer did not identify and evaluate the respiratory hazard in the workplace. Employees used portable right angle grinders to dry grind concrete during finishing operations of the concrete median barrier of the US Highway 36 construction project. Concrete contains crystalline quartz. The dry grinding of concrete potentially released respirable silica. Employee exposure to respirable silica had not been determined. This condition potentially exposed employees to a respiratory hazard.

29 CFR 1910.134(k)(1): The employer did not provide effective, comprehensive, understandable, annual respirator training to employees who are required to use respirators: (a) Concrete Express Inc, at US Highway 36, Louisville, CO: On and preceding 12/3/15, the employer did not provide comprehensive annual respirator training to employees who are required to use respirators. Employees used portable right angle grinders to dry grind concrete during finishing operations of the concrete median barrier of the US Highway 36 construction project. Concrete contains crystalline quartz. The dry grinding of concrete potentially released respirable silica. Employees were required to wear Moldex model 2300N95 or Moldex model 2200N95 filtering facepiece respirators. The employer had not provided annual respiratory protection training. This condition potentially exposed employees to a respiratory hazard.
