Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a)  On or about April 18, 2011 and at times prior, Kelanar Roofing Inc. as the controlling contractor did not ensure that workers were protected from fall hazards.  The subcontractor roofing company was tearing off an old roof from a commercial site.   The workers were exposed to falls of 10-12  while working at the edge of the flat roof.     Abatement Note:  Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
