29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 21, 2018, Kelnar Roofing, Inc., as the controlling contractor, did not ensure A&I Skyline Roofing and other subcontractors, protected their employees from fall hazards while performing roofing work on a residential building roof, in excess of six feet above the lower working surface without the benefit of a fall protection system being in place. (b) On or about November 21, 2018, employees were not protected from fall hazards in excess of 6 feet while performing roofing activities.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about November 21, 2018, employees exposed to falls in excess of 6 feet had not been provided with training in the recognition and prevention of potential fall hazards while conducting roofing activities in excess of six feet above the lower working surface.
