29 CFR 1910.147(d)(4)(ii): Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position. a. Packaging Department- On or about June 21, 2011, the employer did not ensure that lockout devices were used to isolate all energy sources when the employees bypassed a guard on an automated conveyor at the end of the stretch wrapper, thereby exposing employees to struck by hazards. b. Packaging Department - On or about June 21, 2011, the employer did not ensure that lockout devices were used to isolate all energy sources when the employees were cleaning the photo eyes that controlled the conveyor advance system, thereby exposing employees to the potential for re-energization of equipment.
