29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. a. On or about June 21, 2011, at the above addressed jobsite, north and west sides of the roof, warning lines were erected closer than six (6) feet from the roof edge. Employees were thereby exposed to the hazards associated with fourteen (14') feet two (2") inch falls.

On or about June 21, 2011, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of fourteen (14') feet two (2") inches, due to the safety monitor having other responsibilities, such as but not limited to, roofing operations.
