29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fallhazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards. a. On or about November 17, 2011 - at the above addressed jobsite, the employer failed to provide training for an employee exposed to fall hazards while conducting roofing operations.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safetymonitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted. On or about November 17, 2011 - at the above addressed jobsite, an employee conducting flat roofing operations, sixteen (16') feet and eleven (11") inches above the ground, was not protected from falling, thereby exposing the employee to fall hazards. GREAT LAKES ROOFING & CONSTRUCTION CO. INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 45576, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 10, 2007 WITH RESPECT TO A WORKPLACE LOCATED AT 1630 LEWIS AVE SOUTH HOLLAND, IL 60473.
