29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: a) Durez Corporation - Kenton, Ohio: On or about July 5, 2011 the employer did not ensure that a housekeeping program was implemented to prevent the accumulation of combustible phenolic resin dust on equipment and work surfaces in Building 51.

29 CFR 1910.119(d)(3)(i)(C): The process safety information did not contain information pertaining to equipment in the process including electrical classification: a) Durez Corporation - Kenton, Ohio: On or about July 5, 2011 the employer did not ensure that the electrical classification diagram included properly classified hazardous locations for the area of the T-2007 tank. This areas was listed as general purpose/non-hazardous, instead of a Class I, Division 2 location.
