29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not constructed to prevent electrical shock and were not clearly marked for identification of functions.    a) Bay #1 - overhead crane had its pendant control box with the protective casing taped up holding it together exposing energized electrical wires.       b) Bay #4 - The Harnischfeger P&H 30 ton crane, serial #CI - 28653, had its pendant control box with the protective sheath detached exposing energized wires, the buttons on the pendant control box were not clearly marked to verify their functions, and the protective casing on the pendant control box was cracked at the top exposing energized electrical wires.      c) Bay #6 - The Harnischfeger P&H 30 ton crane, serial #CI - 28687, had its pendant control box with the protective sheath detached exposing energized wires, the buttons on the pendant control box were not clearly marked to verify their functions, and the protective casing on the pendant control box was cracked at the top exposing energized electrical wires.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.      a) Bay #1 - The employer did not visually inspect the flexible cord on the Delta radial arm saw, the cord had its outer jacket damaged exposing electrical wires.
