29 CFR 1910.335(a)(1)(v): Employees did not wear protective equipment for the eyes or face wherever there was danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion. (a) Bay 3 - On or about January 14, 2025, maintenance employees removing fuses to bring equipment, such as but not limited to the 48-inch slitter M20 banding machine, to a zero-energy state to perform service and maintenance were not provided with an arc rated face shield. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHA's public web site at www.osha.gov: (a) The employer did not report an amputation that was the result of a work-related incident that occurred on or about October 16, 2024, to OSHA within twenty-four (24) hours. No abatement certification or documentation is required for this item.
