29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) BT Construction, Inc. at 2506 W 105th Ct, Westminster, CO 80234: On and before June 28, 2011, the exposing employer did not ensure that each employee working in an excavation that measured approximately 6.5 feet at its deepest location was protected from cave-in. Employees working next to the near vertical excavation wall on the west side were exposed to a crushing hazard by excavation wall cave-in.
