29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide. Employees were exposed to tripping hazards while operating Tape Edge Machines over 2-inch wide ditches filled with conduit that were not properly covered. Abatement documentation is required of this item in accordance with the requirements 29 CFR 1903.19(d).
