29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a) Brothers Lopez Roofing at 1117 18th St, Greeley, CO 80631:  On and before June 29, 2011, the exposing employer did not ensure employees were protected from injury, in that employees were working next to the edge of the roof on a one-story residential home without the use of a fall protection system. The employees were removing flashing on the portion of the flat roof on the north side of the home. This condition exposed employees to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet above the upper landing, and was not secured:    (a)  Brothers Lopez Roofing at 1117 18th St, Greeley, CO 80631:  On and before June 29, 2011, the employer did not ensure that two portable extension ladders, used to access the roof of the residential home, had side rails extending 3 feet above the upper landing, and was secured. Employees accessed the roof by using two extension ladders on the west side of the house. One extension ladder was used to access the 1st level of roof while the other was used to access the top flat portion of the roof. Both ladders were placed less than three feet above the landings. This condition exposed subcontractor employees to a fall hazard of approximately 10 feet to the ground below.
