29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  Brothers Lopez Roofing LLC at 7003 Egyptian Drive, Fort Collins, CO 80525:  On or before September 10, 2011, the employer did not ensure that employees were using safety glasses when using pneumatic nail guns when shingling this house.  Condition exposed employees to the hazard of projectiles to the eyes.    Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    a)  Brothers Lopez Roofing LLC at 7003 Eqyptian Drive, Fort Collins, CO 80525:  On or before September 10, 2011, the employer did not ensure that employees were using fall protection when shingling a two story residential roof with a 5 in 12 pitch.  Condition exposed employees to fall hazards of up to 20 feet.    Abatement note:  Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").
