29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     (a) 1430 Sanders Rd., Northbrook, Illinois: The employer did not provide a safe access for employees to use when they were working from tubular welded frame scaffold platforms. The employees were exposed to a fall hazard of approximately 16 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Neupauer Masonry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i) which was contained in inspection number 3142110372, citation number 02, item 03, issued on May 15, 2008 and was affirmed as a final order on or about June 3, 2008, with respect to a jobsite located at 8932 Major in Morton Grove, IL 60053.    Neupauer Masonry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i) which was contained in inspection number 310637301, citation number 01, item 03, issued on January 8, 2007 and was affirmed as a final order on or about February 20, 2007, with respect to a jobsite located at 9200 Washington in Niles, IL 60714.
