(a) 1430 Sanders Rd., Northbrook, Illinois: The employer did not provide a safe access for employees to use when they were working from tubular welded frame scaffold platforms. The employees were exposed to a fall hazard of approximately 16 feet. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
