29 CFR 1910.1200(h)(1): \r\n\r\nEmployees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)\r\n\r\nOn July 13, 2011, employees had not received Hazard Communication training on hexavalent chromium which is in Portland Cement, an ingredient in concrete.\r\n\r\n

29 CFR 1926.95(a):     Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On August 3, 2011, the use of gloves was not enforced for at least four employees who were finishing wet concrete.
