From June 27, 2011 to July 8, 2011 during the flashing of roof curbs with 4 foot by 4 foot openings, the employer did not ensure employees were protected from falls of approximately 35.8 feet by using fall protection systems/devices.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fallhazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards. From June 27, 2011 to July 8, 2011 during the flashing of roof curbs with 4 foot by 4 foot openings, the employer did not ensure employees were trained to identify and correct fall hazards when working around the roof openings; thus exposing employees to falls of approximately 35.8 feet.
