On August 2, 2011, the tailpipe emissions of the Nissan lift truck #152 were more than 5% carbon monoxide and the lift truck had not been taken out of service.\\r\\n\\r\\n2. On August 2, 2011, the tailpipe emissions of the Toyota lift truck #5321 were more than 5% carbon monoxide and the lift truck had not been taken out of service.\\r\\n\\r\\n3. On August 2, 2011, the tailpipe emissions of the Nissan lift truck #134 were more than 2% carbon monoxide and the lift truck had not been taken out of service.\\r\\n\\r\\n4. On August 2, 2011, the tailpipe emissions of the Toyota lift truck #97055 were more than 2% carbon monoxide and the lift truck had not been taken out of service.\\r\\n

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals intheir work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trainedabout is introduced into their work area: As of August 2, 2011, employees exposed to carbon monoxide in the tailpipe emissions of lift trucks were not trained on the health effects of carbon monoxide.
