For example, but not limited to type of equipment used to lockout a lever isolation ball valve.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard. (a) Employee(s) used a Milwaukee Grinder (Serial No. B64AD09330638) without a safety wheel guard in place, exposing employee(s) to a struck by hazard.
