29 CFR 1910.146(d)(5)(i): The employer did not test conditions in the permit space to determine if acceptable entry conditions exist before entry is authorized to begin:    a) American Steel Treating, Inc. - Perrysburg, Ohio: On or about August 12, 2011, the employer failed to ensure that all atmospheric hazards were tested for when employees entered the D furnace permit-required confined space. The gas meter the employer used for monitoring tested for hydrogen sulfide, carbon monoxide, oxygen, and combustible gases; however, the furnace atmosphere also contained ammonia, which was not monitored for prior to entry.
