The cord was used to power a Mag 77 Skilsaw, and a Rolair air compressor model 6820K170001, serial number 10010791.  Both tools were not double insulated.  The cord was plugged into a temporary panel with a ground fault circuit interrupter (GFCI).  Condition exposed employees to electrical shock hazards.    Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.502(b)(3):  Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    a)  Fast Framing Inc. at 8581 East 21st Avenue, Denver, CO 80238:  On or before 7/30/11, the employer did not ensure that the guardrails around the stairway openings on both the main floor and the second floor were capable of withstanding a force of 200 pounds.  The horizontal guardrails consisted of construction grade 2 x 4's with posts that were 10.5 feet apart.  Conditions exposed employees to fall hazards of up to 20 feet to the basement.     Abatement note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").    Abatement note:  1926 Subpart M Appendix B provides that the posts shall be spaced not more than 8 feet apart.
