(a) Located in the SE corner of the warehouse, circuit breaker panel MDP did not have a door to provide shielding in the event of a ciruit breaker malfunction.

(a) Located at the Oregon Army National Guard USPFO at Camp Withycombe, on or about July 29, 2011, the employer failed to provide copies of the injury and illness records to an authorized representative. OSHA 300 logs and OSHA 300a summary pages were never provided for 2008, 2009, and 2010. The OSHA 300 log for 2011 also was not provided.

29 CFR 1910.178(a)(5): When trucks are equipped with front-end attachments other than factory installed attachments, it was not requested that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. (a) Located in the USPFO warehouse at Camp Withycombe, a US Army Hyster forklift WJ04T3 was available for use with 6' fork extensions, but the forklift was not marked to indicate the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.
