29 CFR 1910.23(a)(8)(ii):     Floor hole(s), into which persons could accidentally walk, were not guarded by floor hole covers of standard strength and construction:     a)  In Mechanical Room E114, a floor drain that measured 3.5 inches X 8 inches X 6 inches deep, was not equipped with a cover.    b)  In Food Service, Room A100A, a floor drain that measured 3.5 inches X 5 nches X 6 inches deep, was not equipped with a cover.

29 CFR 1910.219(f)(3):     Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed:    a) In the Transitional Care Unit, at the Loading Dock, on the overhead door the drive  sprocket wheel and chain, within 6 feet of the stairs tread, were not enclosed.

29 CFR 1910.23(a)(5):     Infrequently used pit or trap door floor opening(s) were not guarded by floor opening covers of standard strength and construction:    a)  In the Mechanical Room, on a sump-pump-pit, the cover was removed and was not replaced exposing an opening that measured 29 inches X 40 inches X 42 inches deep.    Notice #96968, Item #2-1, 29 CFR 1904.30(a) as required by 29 CFR 1960.8(b) : The Veterans Affairs failed to guard a pit or trap door floor opening. The employer was cited for a violation of the same standard and/or same hazard on 12/17/2007, Inspection #305941569, date 1/14/2008 of final order/settlement, and means of abatement.
