The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards,shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.\r\n\r\na.  In the Maintenance Shop, the point of operation for the Bridgeport Milling Machine (no model or serial number) was not guarded, thus exposing employees to a struck-by hazard.\r\n\r\nb.  In the Maintenance Shop, the point of operation for the Victor Milling Machine, model CD-3VS, serial 451346, was not guarded, thus exposing employees to a struck-by hazard.

29 CFR 1910.305(g)(1)(iv)(A): \r\n\r\nFlexible cords and cables cannot be used as a substitute for the fixed wiring of a structure;\r\n\r\na.  In the Coupling Cutting Area, extension cords were used to power two computerized Shop View System stations, thus exposing employees to electrical fire hazards.\r\n\r\nb.  In the Warehouse, the Orion shrink wrap machine, model LPD, serial 2010-02-V01121, was powered by a flexible cord and plug, thus exposiing employees to electrical fire hazards.\r\n\r\nc.  In the Shipping Area, the International Packaging Machine Inc. shrink wrap machine, serial 89-2847, was powered by a cord and plug, thus exposing employees to electrical fire hazards.
