An effective Hearing Conservation Program shall include at least:    1.	Periodic monitoring and monitoring for changes in the process;     2.	Baseline and annual audiometric testing;     3.	Hearing protectors in a variety of types that is required to be worn; and    4.	Annual training for the hazard of noise and use of hearing protection devices:    a.	The effects of noise on hearing;  b.	The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instruction on selection, fitting, use, and care; and  c.	The purpose of audiometric testing and an explanation of the test procedures.
