29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    a)  On or about August 5, 2011 and at times prior, employees were exposed to injury while working around ironer machines (6) that did not have the rotating steel shafts at the ends of the rollers guarded.  This includes the upper rollers on these machines which rotate at a higher rate of speed as well as the lower rollers where employees are stationed to place laundry into the machines.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):    a)  On or about August 5, 2011 and at times prior, an door to the outside of the building from the ironer area was misslabeled with an "exit" sign, when the door was not being used as an exit.  This door was completely blocked by material.
