The procedure references "pins" for the crosshead without identifying the change in blocking device and a specific location when an I-beam was placed on either side of that component.    b.)  The procedure references pins for the clamp, without identifying the sequence to check that they were properly installed.    c.)  The procedure does not identify the process or requirement for an orderly check that all lockout elements had been implemented correctly during a group lockout process.

29 CFR 1910.147(c)(7)(i)(A):     Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:      a.)  Up to and including 8/10/2011, the Harris shear work crew responsible for changing shear blades and other regular service were not trained in the potential for immediate release of the clamp when the liner bunched up and potentially interfered with the sensor for the bypass hydraulic pressure system.  They also were not trained in evaluating the potential hazard of interfering with safety systems when maintaining lockout overnight.    b.)  Authorized employees were not trained in the lockout checklist.  They signed off on a lockout checklist that did not specify procedures performed, did not verify actions taken, and were incomplete in many instances (# employees did not match #signatures, only portions were checked off, etc.).  Employees were not trained to complete the checklist as an affirmation that all elements of the lockout had been initiated.

29 CFR 1910.147(c)(7)(iii)(A):     Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures:      Employees performing lock out during regular servicing of the Harris (HSC) shear were not retrained on potential hazards when the equipment was modified:    a.)  I-beams were used in lieu of safety pins to block the crosshead;  b.)  The liners were bent and crumpled in the clamp slide, and bolts were replaced with welded components.  c.)  Employees were not separately instructed on which piece of equipment needed a particular block.     d.)  Employees were not trained in the possible hazards of liner modification or removal.
