29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board: (a) A Harris BSH-1025 ton single shear did not have the work platform located at the top of the shear fully guarded to prevent employees from falling onto the in-feed deck of the shear. On or about 7/15/13, an employee fell from the top of the shear while tightening bolts on a hydraulic line. The employee fell approximately 10' onto the in-feed deck of the shear. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) A Harris BSH-1025 ton single shear did not have any form of machine guarding on the ground level to prevent employees from accessing the danger area of the machine when the in-feed deck was loaded and operated, exposing employees to pinch points and rotating parts under the in-feed deck and struck by hazards created from materials that may fall outside of the in-feed deck. ABATEMENT CERTIFICATION REQUIRED
