29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a. On or about June 17, 2011 - at the above addressed jobsite, employees performing underground construction activities in a trench eight(8) feet ten(10) inches in depth were not provided with adequate protective systems. The foreman was exposed to cave in hazards while directing the excavator in a trench with no protective system installed.    THE LEN COX & SONS EXCAVATING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(A)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310171137, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS FINAL ORDER NOVEMBER 29, 2006, WITH RESPECT TO THE WORKSITE LOCATED AT HAMILTON ST. IN LOCKPORT, ILLINOIS 60441.
