29 CFR 1926.652(a)(1):  Each employee in an excavation is not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):     a. On or about July 15, 2020, at the above addressed jobsite, an employee was conducting water main replacement operations in a trench measured at approximately six (6) feet in depth.  The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards.
