29 CFR 1910.119(f)(4):  The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel.  These safe work practices shall apply to employees and contractor employees.    a. Ineos USA LLC located in Lima, Ohio:  On or about July 14, 2011, the employer did not ensure the operator for the purification section field verified with each authorized employee, where the lockout points were located on the spare steam reboiler prior to the authorized employees applying lockout devices to the lockout board. This resulted in approximately 3500 gallons of acrylonitrile being released due to the wrong process line being opened.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
