29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as opening process equipment or piping: On June 10, 2015, at the Heads Column, DA-0501 at the INEOS Nitriles Green Lake facility, the employer's implementation of its Opening Lines procedure relating to hazardous concentrations of hazardous chemicals in process piping did not fully recognize the potential hazard of the operation.

29 CFR 1910.134(d)(2)(I): The employer shall provide the following respirators for employee use in IDLH atmospheres: (A) A full facepiece pressure demand SCBA certified by NIOSH for a minimum service life of thirty minutes, or (B) A combination full facepiece pressure demand supplied-air respirator (SAR) with auxiliary self-contained air supply: On June 10, 2015, the employee monitoring hydrogen cyanide levels in response to an alarm at or near the Heads Column, DA-0501 at the INEOS Nitriles Green Lake Facility, where a reasonable estimate of the employee's exposure to hydrogen cyanide could not be made, did not wear a respirator approved for use in presumed IDLH at atmosphere.

29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation did not exist: On June 10, 2015, the employer did not verify the isolation of hydrogen cyanide until the possibility of such accumulation did not exist during installation and removal of blinds from process piping at the Heads Column, DA-0501 at the Ineos Nitrile, Green Lake facility.
