29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. Salenbien Trucking and Excavating, Inc. on or about August 19, 2011 at the site located at 5812 Main St. in Sylvania, OH: The employer failed to ensure a safe means of egress such as a ladder, was provided to an employee locating water service lines in 4.9 foot deep trench. Salenbien Trucking and Excavating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.651(c)(2), which was contained in OSHA inspection number 313782864, citation number 1, item number 1, and was affirmed as a final order on June 16, 2010, with respect to a workplace located at 8440 Sylvania-Metamora Rd., Sylvania, Ohio. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
