29 CFR 1910.22(d)(1):  On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed to the building:          The (wooden) mezzanine, located in the packing room of the facility, and used for the purpose of storage was not mark or the loads approved by a qualified person.

29 CFR 1910.26(a)(3)(vii):     The bottoms of the four rails are to be supplied with insulating non-slip material for the safety of the user.        Several of the portable metal ladder(s) located throughout the site, were missing the insulating non-slip 	material on the bottom of the rails, which resulted in the ladder(s) becoming unbalanced, thus exposing users to a fall hazard.

1910.178(q)(6): Industrial trucks will not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor will they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks will not be done unless approved by the truck manufacturer.          The employer failed to obtain the consent of the manufacturer before adding a fork-mountable work platform to the  company's Clark, (electric) fork truck.

1910.212(a) (1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.        As of 08/09/2011, the employer did not ensure that various equipment or machinery used at the site was affectively guarded.  The unguarded equipment includes but is not limited to,        a.  The chuck of the Cleveland (vertical) boring mill where the revolving tooling is located, was unguarded and did not           protect operators from hazards created by point of operation or rotating parts.    b. The chuck of the Logan (horizontal) boring lathe was unguarded and did not protect operators from hazards created       by point of operation, rotating parts or entanglement hazards.
