29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system:    On or about August 23, 2010, at the address of 1800 Miami Street, Toledo, Ohio, the employer did not ensure the each employee engaged in roofing activities on a flat roof, with unprotected sides and edges 6 feet more above the lower level was protected from falling.  The employer did not have any form off fall protection in place, exposing employees to falls of approximately 30 feet to the paved ground below, which could result in permanent disability or death.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:    On or about August 23, 2011, at address 1800 Miami Street, Toledo, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations.  Employees were exposed to falls of approximately 30 feet to the paved ground below, which could result in permanent disability or death.
