29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a.) 3857 Amhurst Drive, Hermitage, PA.: Residential roof- on or about 06/10/2022 and at times prior thereto, employees were conducting roofing operations on a roof with an 6:12 pitch without any fall protection systems. Employees were exposed to a fall hazard of at least 10 feet. Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a.) 3857 Amhurst Drive, Hermitage, PA.: Residential roof- on or about 06/10/2022 and at times prior thereto, the Werner metal ladder that was utilized to access the roof was only 6-inches above the roof line. b.) 3857 Amhurst Drive, Hermitage, PA.: Residential roof- on or about 06/10/2022 and at times prior thereto, an extension ladder was setup and extended only 1 foot above the roof line. Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a.) 3857 Amhurst Drive, Hermitage, PA.: on or about 06/10/2022 and at times prior thereto, employees were exposed to poor housekeeping conditions throughout the jobsite. Roofing debris including but not limited to, shingles, nails, and boards were located on the ground throughout the walking/working areas and around the ladder access to the residential roof. Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
