CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     	(a)	SteelStar Corporation, 101 Miller Drive, Dacono, CO 80514:  On and before August 30, 2011, the controlling and exposing employer did not ensure that all affected employees utilizing a Peddinghaus Automated Drilling System utilzing 6 to 9 bits to drill into large structural steal beams, located in the main machine shop, were guarded from hazards created by the machines rotating parts and flying chips and sparks. This condition exposed employees to hazards from the machines rotating parts and hazards from chips and/or sparks while servicing or utilizing the drill bits in the main shop. 29 CFR 1910.212(a)(3)(ii):  The employer failed to ensure that point of operation of machines whose operation exposes an employee to injury were guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle;    (a) SteelStar Corporation, 101 Miller Drive, Dacono, CO 80514: On and before August 30, 2011, the exposing employer did not ensure that the point of operation on a Piranha Multi Station Sheet metal forming machine was adequately guarded.  Specifically, personnel were seen extracting pieces manually from the rear of the shear cutter portion of the machine.  Condition exposed employees to point of operation hazards, laceration and amputation hazards while using the shear press portion of the Piranha multi station metal forming machine.
