Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees are exposed to struck-by and crushing hazards which could inflict death or serious injury while handling granite slabs:  (a)  CFI, LLC at 5561 Magnolia Street, Commerce City, CO 80022:  On and before September 1, 2011, the employer did not follow safe slab handling procedures while transporting and maneuvering granite slabs.  One employee was observed standing in the fall zone of the granite slab while moving a granite slab from the delivery truck to the storage rack.  This employee was observed standing in the truck bed during attachment of the slab to the forklift.  This condition exposed employees to struck-by and crushing hazards.    (b)  CFI, LLC at 5561 Magnolia Street, Commerce City, CO 80022:  On and before September 1, 2011, the employer did not follow safe slab handling procedures while transporting and maneuvering granite slabs.  One employee was observed standing in the fall zone of the granite slab during placement of the slab on the bridge saw table with the forklift.  Two employees were also observed on two separate occasions lifting and moving the granite slabs manually during fabrication operations.  These employees stood next to the bridge saw while manually lowering the granite slab down onto the bridge saw table.  These employees also hand-carried the granite slabs from the bridge saw to the polishing table.  These conditions exposed employees to struck-by and crushing hazards.    Abatement Note:  Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following:  (1)  A slab wedge can be used to separate the granite slabs from one another by having two employees stand to the side of the slab and insert the wedge so that a clamping device can be placed onto the slab to be moved.  (2)  Institute and enforce a policy in which employees are prohibited from standing or putting any portion of their body in the fall zone of slabs.  (3)  Conduct employee training of safe handling procedures to ensure they fully understand the dangers of putting any portion of their body to the side of a moving and/or unsecured slab and the safe methods of maneuvering slabs. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:  (a)  CFI, LLC at 5561 Magnolia Street, Commerce City, CO 80022:  On and before September 1, 2011, the employer did not ensure that granite slabs stored on A frames were secured from falling.  One storage rack was found to have 5 slabs of granite being stored on it and secured with an electrical cord.  This condition exposed employees to a struck-by hazard. 29 CFR 1910.176(q)(1):  A power-operated industrial truck not in safe operating condition was not removed from service:  (a)  CFI, LLC at 5561 Magnolia Street, Commerce City, CO 80022:  On and before September 1, 2011, the employer did not remove a forklift from service that was not in safe operating condition in that the Clark forklift was in found in operation with a defective rear tire.  The tire was missing the outer rubber shell exposing a frayed inner shell.  Employees were operating the forklift to transport granite slabs.  This condition exposed employees to struck-by hazards.
