29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about September 2, 2011, at the above addressed jobsite, an employee was installing a sewer in an excavation measured at six (6) feet six (6) inches in depth. The excavation was not provided with cave in protection, thereby exposing the employee to cave in hazards. THE UNIQUE PLUMBING CO., INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312594997, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 25, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT FILMORE STREET AND 1ST AVENUE, MAYWOOD, IL 60153.
