29 CFR 1926.652(a)(1): Each employee in an excavation is not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a. On or about June 27, 2023, at the above addressed jobsite, employees performing water connection operations in a trench that measured approximately seven (7) feet four (4) inches in depth by approximately four (4) feet seven (7) inches in width on the north end of the trench, were not provided with cave in protection, thereby exposing the employees to cave in hazards.
