(a)J&L Roofing @ 6126 Claire Ct., Fort Collins, CO 80525: On and before August 19th, 2011, the exposing employer did not ensure that fall protection systems were utilized by an employee engaged in roofing activities while working at the unprotected edges of a roof on a single-family residential housing. Employees were exposed to fall hazards of approximately 12'.
