(a)	Southern Foods Group, LLC Dba Dillon Dairy at 5512 Leetsdale Dr., Denver, CO 80246: On and before 9/13/11, the employer did not ensure that standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level. This condition exposed employees to a fall hazard in the milk receiving area. (b)	Southern Foods Group, LLC Dba Dillon Dairy at 5512 Leetsdale Dr., Denver, CO 80246: On and before 9/13/11, the employer did not install and maintain an operable ammonia alarm system that warns employees of an ammonia release. This condition exposed employees to inhalation hazards of ammonia. Adjustment of guard shall be such that pieces of an accidentally broken wheel will be deflected away from the operator. The electrical pallet jack is located at a wet and damp area where the crate washing equipment is. This condition exposed employees to electrical hazards.
