29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: J.B. & Co., Inc., conducting work at 1515 E. Wooster St., Bowling Green, OH: On or about September 13, 2011, the employer failed to ensure that employees received comprehensive training on the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while repairing the roof of the three story apartment building. Employees did not use fall protection and were exposed to approximately 27 feet fall hazard from the roof to the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: J.B. & Co., Inc., conducting work at 1515 E. Wooster St., Bowling Green, OH: On or about September 13, 2011, the employer did not ensure that an extension ladder, used to access the roof of the three story apartment building, was extended at least 3 feet above the roof edge. The employees were exposed to approximately a 27 foot fall hazard when using the ladder to access the working level in order to conduct roofing activities. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Wooster St., Bowling Green, OH: On or about September 13, 2011, the employer did not ensure that employees were protected from fall hazards while repairing a low slope roof of the three story apartment building. Employees were exposed to approximately a 27 foot fall hazard from the roof to the ground below. J.B. & Co., Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10)), which was contained in OSHA Inspection Number 313784498, citation number 1, item number 1, and was affirmed as a final order on October 20, 2010, with respect to a workplace located at 2707 Tracy Road, Northwood, OH. J.B. & Co., Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10)), which was contained in OSHA Inspection Number 311605885, citation number 1, item number 1, and was affirmed as a final order on December 1, 2008, with respect to a workplace located at 200 E. Mansfield St., Bucyrus, OH. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
