29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or safety monitoring systems: A: On or about 9/14/11 during roofing activities (spraying of foam insulation), the employer did not ensure employees used fall protection devices/systems; thus employees were exposed to falls in excess of 25 feet. B: On or about 9/14/11 during roofing activities (disposing of roofing materials), the employer did not ensure employees used fall protection devices/systems; thus employees were exposed to falls in excess of 25 feet.
