29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) or (c) of this section: a) ATH Specialties & Sales at 613 Grand Ave., Grand Lake, CO 80447: On or before September 14, 2011, the employer did not ensure that employees working in a 6.5 foot deep trench, with vertical walls, were using either shoring, or adequate sloping or benching when connecting sewer pipe to a new precast manhole. Condition exposed employees to cave-in hazards. Abatement note: Abatement certification and documentation are required for this item (see enclosed, "Certification of Corrective Action Worksheet"). ATH Specialties & Sales was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 652(a)(1), which was contained in OSHA inspection number 314657677, citation number 1, item 1c, issued July 20, 2010, and became a final order on August 22, 2010, with respect to a workplace located at County Road 533, Breckinridge, CO 80424.
