29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:  a)  ATH Specialties & Sales Inc. at 50 Vasquez Road, Winter Park, CO 80482:  On or before August 17, 2016, the employer did not ensure that employees working in a 5.75' to 7.5' deep trench had a safe means of egress.  The employer had connected a 4 inch residential sewer line to a main sewer line in a trench measuring 19' wide x 45' long.  Two employees were backfilling and compacting the soil using motorized compactors (jumping jacks) on the trench base.  No stairway, ladder, or ramp was in use.  This condition exposed employees to cave-in hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  a)  ATH Specialties & Sales Inc. at 50 Vasquez Road, Winter Park, CO 80482:  On or before August 17, 2016, the employer did not ensure that employees working in a 5.75 to 7.50 foot deep trench were using adequate sloping, benching, or shielding while backfilling and compacting the soil in a trench.  The employer had connected a 4 inch residential sewer line to a main sewer line, and was manually compacting the backfilled soil for the road base.  The employer did not adequately slope the north and south walls when the employees were working in the trench.  Condition exposed employees to cave-in hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  ATH Specialties & Sales Inc. at 50 Vasquez Road, Winter Park, CO 80482:  On or before August 17, 2016, the employer did not provide chemical hazard communication training for the employees.  The crew connected a 4" residential sewer line to an existing main sewer line.  In this process, employees were exposed to chemical hazards associated with the primer and glue used to make the pipe connections.  Condition exposed employees to chemical hazards caused by the lack of chemical hazard training.
