The employer had connected a 4 inch residential sewer line to a main sewer line in a trench measuring 19' wide x 45' long.  Two employees were backfilling and compacting the soil using motorized compactors (jumping jacks) on the trench base.  No stairway, ladder, or ramp was in use.  This condition exposed employees to cave-in hazards.

The employer had connected a 4 inch residential sewer line to a main sewer line, and was manually compacting the backfilled soil for the road base.  The employer did not adequately slope the north and south walls when the employees were working in the trench.  Condition exposed employees to cave-in hazards.

Condition exposed employees to chemical hazards caused by the lack of chemical hazard training.
