Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted.    a) Cosam Contracting, Inc. worksite located at 241 E. Mansfield in Bucyrus, Ohio:  On or about September 15, 2011, the employer did not assure employees were protected from falls while performing tasks associated with roofing.  Employees performing roofing operations on a commercial roof were exposed to falls of approximately 19 feet and up to approximately 24 feet on the unprotected edges of the roof.      Cosam Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 311611321, citation number 1, item number 1 and was affirmed as a final order on November 4, 2010, with respect to a workplace located at 238 N. Sandusky Ave., Bucyrus, Ohio.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards.    a) Cosam Contracting, Inc. worksite located at 241 E. Mansfield in Bucyrus, Ohio:  On or about September 15, 2011, the employer did not assure that employees were trained to recognize appropriate use of fall protection.  A parapet wall was inadequate in providing fall protection, exposing employees performing roofing operations to fall hazards of approximately 19 feet and/or up to approximately 24 feet at the unprotected edges of the roof.      Cosam Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.503(a)(1), which was contained in OSHA inspection number 311611321, citation number 1, item number 2 and was affirmed as a final order on November 4, 2009, with respect to a workplace located at 238 N.
