Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Cosam Contracting, Inc. on or about July 29, 2013, at the address of 18 Allison Dr. Shelby, OH: The employer did not ensure employees were protected from falls while performing roofing work on a low-slope commercial structure. Employees were exposed to falls of up to 14.5 ft. to the ground below.
