29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Bldg D:    1)  The employees, walking on the tops of the 3rd floor ceiling joists without fall protection, were not adequately trained in the recognition of fall hazards and the procedures used to control them.    2)  The employees, accessing upper levels via the stairway at the south end of the building and exposed to falling from the unprotected doorway at its base, were not adequately trained in the recognition of fall hazards and the procedures used to control them.

29 CFR 1926.1053(b)(1):  Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.     Bldg. D:  The 6 foot step ladder, used by the employees to descend from the attic space, did not extend 3 feet above the landing surface.
